If someone with multiple personalities threatens to kill herself, is it considered a hostage situation?

As a general rule, the "real" personality is completely unaware of the others. The hallicuinatory personalities are aware of the others, but view them as "normal" other people.

Of course, if Cindy Sheenan WERE to oblige us, I don't foresee much disappointment.
